What do you need to know?

  • ORI #: The church must have its own ORI #, Mail Code, and Billing Code assigned by the Department of Justice. (If you do not have an account established with the Department of Justice, visit www.unbservices.com for information and assistance on how to apply and get set up).

  • New Law Requires that ALL employees and any volunteer who works with minors (ages 0-17) for a minimum of 16hrs/month or 32hrs/year be Live Scanned.

  • Screening Requirements: It requires DOJ and FBI screening.

  • Additional fees: The DOJ does not charge a fee for volunteers. The FBI charges $15 per volunteer and $17 per employee. This fee will be billed directly to the church when it is included on the Request for Live Scan form. If the billing number is not included, the District will bill those fees at the time you are invoiced for the rolling fees.

  • Payment for Services: The District will invoice the church the week after the Live Scan service is provided based on the number of people screened.

  • What Does the Person Being Scanned Need: Each person being Live Scanned will need to provide a completed Request for Live Scan form and have a California Drivers License or California Photo I.D. with them. Please note that anyone with an expired I.D. will not be able to be scanned.

  • How to Fill out the Request for Live Scan Form: Here is a sample of the Request for Live Scan form, what section the church needs to complete, and what fields the individual being scanned needs to complete. Click here for Sample

  • How often does someone need to be Live Scanned? One time. Part of the Live Scan process includes subsequent arrest notifications. If anyone who has been Live Scanned should have an incident in the future, the Custodian of Records will be notified through the Department of Justice portal. That is why it is important that y our Custodian of Records checks the portal on a regular basis.
